Department Case ManagerJOB SUMMARYWorks within a medical emergency
department and medical floor to provide support and discharge
planning of identified patients. Focus on expertise in discharge
planning with extensive knowledge of behavioral health resources.
Days of work are M-F with flexibility required at high volume
times. Must demonstrate the ability to work within high stress
environment, work independently, and function within the behavioral
health treatment team.1. Participates as an active member of the
behavioral team.
- Begins and ends workday as scheduled and is willing to work
flexible hours within the days.
- Integrates effectively into the emergency department
environment and works well with UTMC staff.
- Responds appropriately to all flags, emails, and
voicemails.
- Demonstrates a consideration and concern for fellow workers and
their jobs and promotes harmonious relationships and
attitudes.
- Is expected to have regular and predictable attendance and the
ability to work cooperatively with others.
- Attends and participates in scheduled administrative team
meetings.
- Accepts additional assignments and/or changes in assignments
and/or work.
- Must stay awake and alert all shifts.2. Completes documentation
of clients in compliance with licensure and funder standards.
- Completes all necessary documentation for each client
daily.
- Clearly document clinically relevant information.
3. Provides appropriate and individualized discharge planning.
- Acts as a liaison with community agencies and families to
ensure access to care for client.
- Communicates with all crisis programs (MCU, CSU/WIC) and other
behavioral health programming in order to develop an appropriate
individualized discharge plan for identified clients.
- Works effectively with ED Master's level staff and UTMC staff
prioritize need.

- Starting salary for this position is approximately $20.26 /hr
based on relevant experience and education.Schedule:
- Due to the diversity of job responsibilities, this position
requires flexibility in scheduling (days, evenings, and
weekends).Travel:
- Must be capable of driving a van and also transporting clients
in personal vehicle.QUALIFICATIONS - University of Tennessee
Medical Center Emergency Department Case ManagerExperience /
Skills:
- One-year practical experience working with individuals with a
mental illness preferred.
- Must have a strong commitment to the right and ability of each
person with a mental illness to live and work in the community and
to have access to competent and appropriate support services.
- Must have demonstrated ability to abide by professional/ethical
code of conduct and to establish supportive and respectful
relationships with clients.Education / Knowledge:
- Bachelor's degree in a behavioral health field of counseling,
psychology, social work, sociology, or other behavioral science
field with course work or experience in the areas of cultural
diversity, human development, etiology and treatment of mental
illness, alcohol and drug abuse, physical and sexual abuse, suicide
and intellectual and developmental disabilities.Physical:
- Possible exposure to biological hazards.
- Lifting up to 50 lbs.
- Pushing/pulling up to 150 lbs.
- Frequent sitting, standing, walking, bending, stooping, and
reaching.
- CPR & First Aid required.Location:
