Physical Therapist Location: Gainesville, GA Agency: Cynet Health Pay: $2,440 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 1/19/2026 About the Position Job Title: Physical Therapist - Home Health Profession: Therapy/Rehabilitation Specialty: Physical Therapy Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Recent Home Health experience Description: Home visits required for therapy services. Periodic office meetings are expected. Reliable transportation is necessary. Current driver's license and agency-required liability insurance are mandatory. Average number of patients per day: 5-6. Productivity expectations: 30 points per week. Contractor will be assigned between 5-10 patients per day. Must be willing to travel up to 75 miles from home clinic. Mileage is reimbursed at the IRS standard rate. The commute to and from the first and last patient are not billable. Weekend requirements vary by location and profession. Types of cases include start of care and follow-up visits. Candidates must be comfortable with driving to patient homes daily, as well as to the clinic for weekly meetings. All documentation must be completed within 24 hours of patient appointments. There are no guaranteed hours with this contract. Clinician must meet weekly productivity points of 30, which equates to a standard workweek. Declining patients may lead to canceled assignments. Clinicians must be open to on-call rotations. Local candidates are accepted. All travelers must be proficient in specified EMR systems. The expected travel distance may result in drives longer than 20 minutes between patients.
