- Telephonic Pharmacist to join growing healthcare system based out
of Atlanta, GA! Description Under supervision of a Clinical
Pharmacy Assistant Manager as designated, supports Formulary
adherence by providing consistent medication review and benefit
coverage determination. Assist new members with the transition of
chronic medications into Kaiser Permanente, facilitating
appropriate medication conversions. Partner with practitioners to
optimize medication regimens. Review acceptance of conversion
recommendations associated with new member consults. Maintain
pharmacy services in accordance with departmental policies and
standards as well as state and federal laws and regulations. Works
in concert with medical office and regional office administration
to ensure effective delivery of pharmacy services. Interacts with
practitioners, peers and patients in order to effectively explain
and shape prescribing practice and formulary adherence. Assist with
achievement of strategic initiatives and critical areas of pharmacy
focus (i.e. - Formulary adherence, medication reconciliation, &
criteria restricted medicine use). Able to effectively navigate
electronic resources to research, influence and support drug
benefit coverage decisions. Comprehends and utilizes policies,
procedures, protocols, drug information resources and TSPMG
treatment guidelines as appropriate. Interacts and consults with
practitioners when necessary to improve quality of care and improve
value; educates practitioners regarding cost effective therapy and
drug utilization goals (e.g. preferred drug, formulary and drug
promotion). Specific responsibilities may include: assessing new
member medication regimens, identifying HRME, assessing ADEs,
patient enrollment into specific services, educating patients
and/or caregivers regarding drug therapy and disease management,
coordinating drug therapy with home health care services,
telecommunication with patients and practitioners, notifying
patients regarding dosage adjustments, scheduling lab follow-up
appointments, designing drug treatment plans for patients, managing
adverse events and drug interactions, using computerized pharmacy
databases for drug information, patient tracking and documentation,
generating consultations to practitioners and documenting in
medical record. Provides pharmaceutical care for members including
prospective, concurrent and retrospective drug utilization review,
medication reconciliation, screening for drug allergies, monitoring
interactions and duplications, following-up with physicians as
appropriate, and counseling patients. Serves members in a friendly,
courteous and expeditious manner to promote member satisfaction and
retention. Consistently supports compliance and the Principles of
Responsibility (Kaiser Permanente's Code of Conduct) by maintaining
the privacy and confidentiality of information, protecting the
assets of the organization, acting with ethics and integrity,
reporting non-compliance, and adhering to applicable federal, state
and local laws and regulations, accreditation and licenser
requirements (if applicable), and Kaiser Permanente's policies and
procedures. Access to protected health information (PHI) will be
limited to the minimum necessary required to effectively perform
your job. May perform other duties as assigned. Additional Skills &
Qualifications Pharmacist License in Georgia Doctor of Pharmacy
Clinical formulary adherence knowledge Microsoft office - excel
test given High level communication skills - written, typed and
verbal Problem solving skills High level of customer service to
handle potential upset members while advising them PLUS: 1. prior
authorization experience 2. experience working in PBM 3. MTM
experience 4. Lexicomp experience 5. Ambulatory pharmacy experience
6. Experience with EPIC and other EMRs Job Type & Location This is
a Contract position based out of ATLANTA, GA. Pay and Benefits The
pay range for this position is $50.00 - $55.00/hr. Eligibility
requirements apply to some benefits and may depend on your job
classification and length of employment. Benefits are subject to
