In this role, you will have the opportunity to work one-on-one with patients in their homes, helping them achieve their highest levels of physical function and independence. Perform initial patient evaluations to assess physical condition and establish a personalized care plan Regularly visit patients in their homes to perform therapeutic exercises and treatments Monitor patient progress and adjust care plans as necessary Educate patients and family members on treatment plans and in-home therapy techniques Collaborate with other healthcare professionals to coordinate patient care Maintain accurate documentation of treatment and patient progress Adhere to all professional, ethical, and legal regulations as required by law and company policies Strong interpersonal and communication skills Ability to work independently in a home setting Expertise in designing and executing effective physical therapy treatment plans Proficiency in using physical therapy equipment and devices Adaptability to various patient needs and environments Medical Insurance Dental Insurance Vision Insurance 401k Current state license as a Physical Therapist Valid driver’s license and reliable transportation CPR certified Completion of a national physical therapy examination
