In this role, you will work closely with patients in their homes, helping them manage various medical conditions and recover from illnesses or surgeries, all while maintaining their independence and comfort. Conduct initial patient assessments and develop individualized plans of care in collaboration with other healthcare professionals. Administer medications, monitor vital signs, and assist with daily living activities. Educate patients and their families on health conditions and self-care skills. Ensure documentation and compliance with healthcare and safety regulations. Provide emotional support and foster a positive environment for patients and their families. Strong clinical skills in a home health setting Excellent communication and interpersonal skills Ability to work independently and manage time effectively Proficiency in electronic health record (EHR) systems Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid Registered Nurse (RN) license in the state of practice CPR certification Preferably certified in Basic Life Support (BLS)
