a qualified Home Health RN in Marietta, Georgia, 30067! Job Title:

Experience: Minimum 1 year Medical/Surgical Nursing required; Home
Health and ICU experience preferred License: RN - Reg Nurse or RN -
Multi-state Compact Certifications: BLS - Basic Life Support or
equivalent, Valid State Driver's License, Private Vehicle Insurance
Must-Have: Strong critical care nursing skills Computer/data entry
experience Ability to communicate with various members of the
healthcare team Basic knowledge of Excel and Word Description: The
RN Home Health is responsible for providing an initial, resumption,
and recertification evaluation of patients in their home setting.
The role includes routine home health visits with age-appropriate
interventions that meet the patient's physical, emotional,
developmental, psychosocial, and educational needs. This position
encompasses the coordination of initial disciplines to begin care
and planning of care throughout the patient episode. Collaboration
with the physician, patient, family members, and team manager is
essential to develop the expected patient visits. The RN schedules
visits for the appropriate team member and incorporates
therapeutic, preventative, and rehabilitative nursing actions.
Effective communication with all team members, including other RNs,
LPNs, therapists, social workers, home health aides, and others is
paramount to ensure the patient's plan of care is developed
appropriately. Direct patient care responsibilities include
assessing, coordinating with other disciplines, and managing the
patient's total needs while following physician orders. The RN
develops the patient's plan of care in coordination with other
disciplines and performs both routine and complex nursing
procedures. Collaboration with the physician, patient, family, and
team manager is necessary for developing the plan of care. The RN
also develops the care plan provided to home health aides and
maintains communication with the physician to report any changes
that may affect the patient's recovery. Patient and caregiver
education is a vital part of the role to develop and encourage
self-care and achieve positive outcomes. Utilizing verbal and
written tools facilitates learning and promotes independence during
recovery. The RN must report any concerns to authorities or agency
leadership regarding issues that may affect patient or staff
safety. Communication involves reporting patient status changes and
lab results to the physician as appropriate and participating in
care team conferences. The RN is responsible for checking voice
mail and staff messaging a minimum of once daily. Knowledge and
appropriate application of regulatory guidelines are expected, and
accurate completion of required documentation is necessary within
established timeframes. Participation in staff education includes
orienting and educating new employees, attending continuing
education, and remaining current in clinical knowledge for home
health patient care. The RN supports and participates in
performance improvement activities, including chart reviews and
identifying methods for improving department performance. Meeting
established productivity standards, participating in weekend and
call rotation as required, and performing other duties as assigned
by leadership are essential aspects of the role. Compliance with
all policies, standards, and code of conduct is mandatory. 5
