A company is looking for a Health Outreach Associate (Bilingual
Spanish).
Key Responsibilities
Increase patient engagement and retention through compassionate
outreach
Facilitate smooth onboarding processes for new patients
Collaborate with internal teams to ensure a seamless patient
experience from initial contact to ongoing care
Required Qualifications, Training, and Education
Bilingual in Spanish and English
Experience in a healthcare environment, preferably with patients
living with dementia or Alzheimer's
Proven ability to achieve conversion goals and maintain high
patient care standards
Tech-savvy with the ability to troubleshoot technical issues
Must provide a personal computer/laptop that meets security and
telehealth standards
